Objective
To ensure appropriate drug usage according to national/international standards & guidelines.
Principal Accountabilities :
Operational
• To make sure that all the protocols are strictly observed as per JCI & NABH guidelines.
• To make sure that all the work done is as per departmental SOPs.
• To make sure that right drugs (rationale drug prescribing) are dispensed in both IPD & OPD departments through
OPD as well as IPD monitoring of prescriptions.
• To communicate with doctors for the errors whenever there is an error with respect to the drugs at every level of prescription (medication error reporting)
• To coordinate with the health care providers (doctors, nurses, pharmacy) regarding drug usage and their safety (medication management and usage).
• To have a thorough knowledge of generic, brand names and category of all available drugs in the institution.
• To collect, monitor and report any possible adverse events (Pharmacovigilance) occurring by virtue of drug-drug interaction, drug-food interaction, idiosyncratic reaction.
• To classify AE’s according to WHO ADR reporting classification and creating a database of all ADR’s (ICSRs) at Vigiflow software.
• To contribute towards data collection of all antibiotic usage patterns in IPD wards and OT’s for AMR (Antimicrobial Resistance) data collection and endeavoring towards antibiotic stewardship.
• To collect and monitor TAT (Turn Around Time) of all the indents received in the In-patient pharmacy.
• To receive GRN from relevant department as and when required.
Performance Parameters
• Preparation and modification of formulary in accordance to the hospital policy / requirement.
• Monitoring of critical clinical trials.
• Monitoring of drug policies.
Education
Essential: M Pharma / B Pharma / D Pharma
Experience
0-4 years in equivalent role in a Hospital or research institute.
Knowledge
• Dynamic, Energetic and self Motivated
• Strong interpersonal, persuasion and facilitation skills with people of different levels, background and personality styles.
• Strong written and verbal communication skills.
• Excellent presentation and facilitation skills.
• High level of computer literacy with ability to work in a paperless formulary environment.
